Our Care Coordination program puts people first and improves interactions between patients and their doctors. This approach is built upon three cornerstones that are unique to eQHealth:
Embedded Care Coordinators: We believe Care Coordinators should be familiar with the communities they serve. We identify Care Coordinators who live in the community, thus ensuring they develop an active, in-person relationship with program participants.
System Infrastructure: Our web-based application, eQCare, supports the daily activities of each member of the care coordination team. This allows for the person-centered interventions to be easily tracked and measured.
Community-Driven: Before we begin providing services in an area, we first identify and engage the local community leaders. This allows us to better understand the particular needs of a community, its residents and those with chronic health concerns.
